Sorry for the dumb question, but you backed out the set screws on that end of the coupler already, correct? If you look into that end of the couple, the set screws are not extending into the hole at all?

I had the same count:
Would I be ignorant in thinking that the spindle could simply be replaced with an extruder in order to get 3D printing on a Shapeoko? Obviously, adding a hotbed and necessary electronics to drive the extruder, etc would go along with that, but as far as the mechanics go, would that be a correct asse...
I also seem to be short 2 of these. As well as 2 x 3/4" nylon spacers. I just counted what I used, and there are definitely 20 in use, which is what the bag says. 8 - for mounting the base plates to the lower Makerslide sections 2 - for mounting the delrin nut 4 - for mounting the rotary tool mounts...
