It only uses one hole. Is there no single free hole on your Y-Plate? You should be able to use just about any of them as long as the cables are long enough.
I don't know if they changed the plates for batch 4.

Can you explain how it is not gripping (yes, it's supposed to, have you looked at the instruction at http://www.shapeoko.com/wiki/index.php/Assembly_step_9)?
I haven't got a clue what the problem could be from what you've written.

I'd say you can safely file the hole a little bigger, because the exact position of the v-wheels isn't that critical and you can tighten them with just the eccentric spacer which doesn't fit.
