As far as I know you mostly need a fan, that should most likely suffice, but a heatsink alone wouldn't (although they certainly don't hurt when combined with a fan).

What keeps the rotary tool perfectly upright at all times? Wouldn't the slightest bit of resistance cause it to tilt? The grblshield plugs into the Arduino, it's not a standalone controller.

Can't you fix that by measuring how far the machine travelled in one axis when it should have travelled, say, 170mm, and then adjust the number of steps per unit of length accordingly?

That's a known problem. Some MakerSlide is out of spec, one of the holes somehow drifted around inside it... There are several threads on it, if you search a little. I think I've read of some people who succesfully do that with a grblshield.
The cables to both steppers running on the same driver should have about the same length because of something the driver does to microstep, I think.
