Oh man, my worst fear come to life. . .
I have run unattended before, but NEVER with code that has not run without issues. I'll usually run new code a couple of times before I am comfortable enough to walk away from it for 10-15 minutes at a time.

Consider the possibility of the spindle starting a fire by friction and take suitable precautions (having a fire extinguisher handy, &c.).
Monitor the machine while it operates, ensuring there is no build-up of dust, debris or fumes, and that nothing works loose...
Note: Be careful to recycle or safely dispose of milling debris and dust, keeping in mind flammability, (potential) spontaneous combustion, and chemical considerations.

It's actually kind of nice to see just how much time there was between when things first went wrong, to when the smoke started, to when the actual fire started. If the operator had been present they could have easily cut the power the second the clamp failed and only been out a piece of wood.

Exactly HOW was that piece clamped?
(Bubblegum and paperclips doesn't count... )
Also, the operator allowed dust to build up everywhere on the piece. And with the RPM/Feed he's been running, that's a mighty fine dust. I guess they should consider themselves lucky that it didn't go boom...

Since watching that video I've done some google searches that will make me hesitate before using a downcut bit to penetrate all the way through metallic work pieces. It turns out a lot of people have written about the danger and some others have also experienced fire as a result of similar operations. Googling for "downcut bit drill through fire" turns up some of these.
